HOMEMAKER/
PERSONAL CARE (HPC)
Often called HPC, a Homemaker/personal care advances the individual's independence within his/her home and community and helps the individual meet daily living needs.
This waiver gives the person more control and authority to decide who provides their services and how those services are provided. The person must want to be in charge of staff employer responsibilities, however, they can receive assistance.
PARTICIPATE-DIRECTED HPC
(PD-HPC)
OHIO SHARED LIVING
This service provides an option for adults with a developmental disability to get the support they need while living with a family member or a caregiver. The person resides with one or more family members or caregivers, and they provide twenty percent or more of the person's care and support services. Shared Living family members and caregivers support adults with developmental disabilities in a home setting.
HPC transportation is a service that people usually receive from an HPC (homemaker personal care) provider to help them run errands, get to appointments and participate in social activities.
HPC TRANSPORTATION
NON-MEDICAL TRANSPORTATION (NMT)
Typically referred to as NMT, non-medical transportation is commonly used to help people get to and from work, volunteer activities and day program sites.
Medication Administration - training and certification developed by DODD authorizes caregivers to perform a variety of tasks for people with many different medical needs. This has a very detailed 14 hour beginner training and an annual training is required.
MEDICATION ADMINISTRATION
SUPPORTED LIVING
Allows individuals with I/DD to live in private homes or an apartment with three or fewer other people. This program enables individuals to design a custom package of daily living supports, such as HPC, remote monitoring, and nursing, to maximize independence and self-determination.
Service Eligibility Requirements
To become eligible for services an individual must meet the following criteria:
1.
Has been diagnosed with a severe chronic disability, attributable to a mental or physical impairment or combination of physical and mental impairment that is not an impairment caused solely by mental illness.
2.
The disability(ies) is established before the age of 22.
3.
The disability(ies) will continue indefinitely.
